There were two providers one was a clinical physiologist the other had radiological training, but I don't remember the title. They were both excellent, they explained everything step-by-step. I had a time, they answered all my questions, they were kind, gentle, and very professional.

I'd like to say that your questionnaire would benefit from some modifications. By not having a "Not Applicable" on more answers, you force either a "No" response which could be unjustifiably taken as a negative or "Definitely" to avoid a punitive interpretation. Dr Parpos and team are great.
